St. David Athletics
We are enthusiastic about athletics here at St. David Unified School District. It is our mission to give our students the opportunity to participate in healthy sporting activities in a safe, fun, and encouraging atmosphere. We will strive to create activities that are student centered and that display integrity, while being quality-focused and providing a balance in the lives of our students.
Requirements for Student Athletes
Before a student may participate in a sport, you must obtain a "white card" by completing the following requirements. See Mrs. Richardson in the front office for paperwork, payment, and if you have any questions.
Parents and healthcare provider need to complete the physical form. This form is good for the current school year.
Parent and athlete need to complete the sports paperwork packet available in the front office.
Middle school students must complete the AIA concussion test and print the certificate at the end. High school students are now required to take both the concussion and opioid tests once per high school career. Athletes must do this once during middle school for middle school athletes (middle school AIA concussion test instructions) and once during high school for high school athletes (High School AIA Brainbook Concussion Course & Opioid Education test instructions). See grade level appropriate instructions for completing the concussion test.
High school fees: $40 per sport. Middle school fees: $35 per sport.
Bring your completed physical form, concussion certificate, paperwork packet, and payment to our activities secretary in the front office.
Payment Information: Please contact our office to make any payments to the school. Currently only cash or checks are accepted. Make checks payable to St. David Schools.

K–12 Students - $3.00
Over 65 - Free
Under age five (not in school) - Free
Family Pass (All sports) - $140.00
Passholder + 1 - $90.00
Student Pass - $45.00
Under age five (not in school) - Free
Faculty + 1 guest - Free
The Athletics Department may have special events where they waive entrance fees for certain groups for activities.

March 20, 2025 Parent Email Re: 2025-26 SPORTS PHYSICALS
Hello Tiger parents,
Just sending out a second notice that we WILL NOT be able to have Chiricahua come to sports physicals for next year. They have changed their policy and are no longer providing this opportunity. So PLEASE, make it a priority to schedule your student athletes for their 2025-2026 AIA sports physicals to be done anytime from now until prior to the beginning of fall sports. Your doctor's office should have the forms but they are attached here for your convenience.
